Abstract: Objective: To explore the impact of outpatient collaborative nursing and 5A nursing mode health education on the self-management ability and quality of life of patients with coronary heart disease. Method: 150 patients with coronary heart disease who received treatment in an outpatient department of a certain hospital from January 2022 to December 2022 were selected as the research subjects. They were randomly divided into a collaborative group and a conventional group using a random number table method, with 75 patients in each group. The collaborative group adopts a health education intervention combining outpatient collaborative nursing and 5A nursing mode, while the conventional group adopts conventional nursing intervention. Record the scores of the Coronary Heart Disease Self Management Scale (CSMS) for disease medical management, daily life management, and emotional management before and after intervention for two groups of patients, as well as the quality of life scores for psychological, physical, social, and material life functions. Results: After intervention, compared with the conventional group, patients in the collaborative group showed significant improvement in CSMS scores in disease medical management (t=15.771, P=0.001), daily life management (t=12.931, P=0.001), and emotional management (t=13.361, P=0.001); The collaborative group had significantly higher quality of life scores in psychological function (t=20.051, P=0.004), physical function (t=23.084, P=0.004), social function (t=24.314, P=0.004), and material life (t=32.474, P=0.004) compared to the control group, and the difference was statistically significant. Conclusion: The combination of outpatient collaborative nursing and 5A nursing mode in health education can significantly enhance the disease self-management ability of patients with coronary heart disease, optimize their psychological, physical, social, and material life functions, thereby improving their overall quality of life. It is worth promoting and applying in the management of coronary heart disease.
