Observation on the Efficacy of Auricular Holographic Gua Sha with Bronze Scraping Tool in Treating Sleep Disorders among Perimenopausal Women

Abstract: Objective: To evaluate the clinical efficacy of auricular holographic Tongbian Gua Sha therapy in treating sleep disorders among perimenopausal women. Methods: A prospective randomized controlled trial was conducted involving 130 perimenopausal women with sleep disorders recruited from the Hospital of Integrated Traditional Chinese and Western Medicine, Southern Medical University. Participants were randomly assigned to an observation group (n = 65) or a control group (n = 65). Both groups received routine emotional support and sleep hygiene guidance. The observation group additionally received auricular holographic Tongbian Gua Sha therapy once weekly for 6 sessions (3 courses, 2 sessions per course). The intervention was guided by the principles of holistic thinking and syndrome differentiation in traditional Chinese medicine (TCM). Primary acupoints included Uterus, Endocrine, Ovary, Pituitary, Thalamus, Liver, and Kidney; adjunctive points included Heart, Spleen, Sympathetic, and Subcortex. Point selection was individually modified based on TCM patterns such as disharmony between heart and kidney, deficiency of both heart and spleen, liver fire disturbing the heart, and deficiency of heart-gallbladder qi. Each session lasted 30 minutes. Outcomes were assessed at the end of week 6 using the Pittsburgh Sleep Quality Index (PSQI), modified Kupperman Index (KMI), and TCM symptom score. Results: After 6 weeks of intervention, the observation group showed significantly lower scores than the control group in PSQI (8.43 ± 2.76 vs. 20.34 ± 5.92), KMI (8.55 ± 2.58 vs. 12.83 ± 2.67), and TCM symptom score (6.03 ± 1.85 vs. 12.42 ± 1.82), with all differences highly statistically significant (P < 0.001). Using PSQI ≤ 7 as the criterion for clinical efficacy, the effective rate was 43.1% (28/65) in the observation group, significantly higher than 0% (0/65) in the control group (χ² = 32.15, P < 0.001). The intervention was safe: only 5 participants (7.7%) experienced transient auricular erythema, which resolved spontaneously within 24 hours; no serious adverse events occurred. Conclusion: Auricular holographic Tongbian Gua Sha therapy significantly improves sleep quality and alleviates overall perimenopausal symptoms in perimenopausal women. It is a safe, effective, and well-accepted non-pharmacological intervention worthy of clinical promotion and application.
