Long-term Efficacy and Radiation-induced Damage Observation of Concurrent Chemoradiotherapy for Locally Advanced Esophageal Cancer

Abstract: Objective: To analyze the long-term therapeutic efficacy of concurrent chemoradiotherapy (CCRT) in patients with locally advanced unresectable esophageal cancer, and to systematically observe the incidence and distribution characteristics of various radiation-induced injuries during treatment. This study aims to provide clinical references for optimizing comprehensive treatment regimen and formulating injury prevention strategies for locally advanced esophageal cancer. Methods: A retrospective analysis was conducted on 116 patients with locally advanced esophageal cancer admitted to our hospital from January 2020 to December 2023. Based on whether concurrent chemotherapy was administered during radiotherapy, patients were divided into a concurrent group and a sequential group, with 58 cases in each. The concurrent group received a combined CCRT model, while the sequential group received the traditional sequential model of chemotherapy after radiotherapy. Both groups maintained identical radiotherapy protocols, chemotherapy drugs, and dosages. Following the completion of all treatments, a two-year long-term follow-up was conducted. Long-term survival indicators and tumor recurrence/metastasis were compared between the two groups. Meanwhile, the types, grades, and incidence rates of radiation-induced injuries during treatment and the follow-up phase were recorded. Results: The concurrent group demonstrated significantly higher 2-year overall survival (OS) and progression-free survival (PFS) rates, as well as a lower local recurrence rate compared to the sequential group (all P<0.05). Furthermore, there was no statistically significant difference in the incidence of late radiation-induced tissue injuries between the two groups (P>0.05). Conclusion: Concurrent chemoradiotherapy for locally advanced esophageal cancer can effectively improve long-term survival benefits and reduce the risk of tumor recurrence, demonstrating superior comprehensive therapeutic value compared to the sequential regimen. Although this model slightly increases the probability of acute radiation-induced injuries, the risk of severe injuries is not significantly elevated. The severity of injuries can be controlled through targeted clinical interventions, indicating promising prospects for clinical application.
